What Is Causing My Shoulder Pain?

Shoulder pain can be caused by a variety of issues, ranging from minor aches and soreness due to a muscle strain to severe bone-on-bone friction caused by arthritis. It can also develop after an injury, sometimes leaving you feeling like your shoulder is “frozen.”

Shoulder pain can show up in many ways—such as reaching up to the top shelf to put away dishes or having trouble finding a comfortable sleeping position.

Some of the most common causes of shoulder pain include:

  • Impingement: This usually occurs due to abnormal movement of the humeral head when you lift your arm overhead. Pain often appears when raising the arm to 90 degrees or higher.
  • Arthritis: The two main types of arthritis that affect the shoulder are osteoarthritis and rheumatoid arthritis. Both can cause reduced mobility, muscle weakness in the shoulder, and difficulty performing normal daily tasks. Rheumatoid arthritis occurs when the immune system attacks the tissues surrounding the shoulder joint, leading to pain and inflammation. Osteoarthritis develops when cartilage in the shoulder joint undergoes significant “wear and tear,” often due to age or overuse.
  • Tendinitis: This develops when the shoulder joint is overworked—often due to the physical demands of a job, frequent overhead activity, or sports. Poor posture can contribute to tendinitis by altering shoulder alignment, causing strain and weakness that increases the risk of tendon injury. This can lead to ongoing inflammation, swelling, and painful impingement when lifting the arm.
  • Frozen shoulder: This condition is characterized by a painful loss of shoulder movement, along with tightening of the shoulder joint that severely restricts mobility. It is also known as adhesive capsulitis. Frozen shoulder may develop if your arm has been immobilized in a cast or sling for a prolonged period, or if you have been bedridden for an extended time. Certain ethnic groups may be more predisposed, and women between the ages of 40 and 60 tend to experience this condition more frequently than men.
  • Rotator cuff injuries: Also known as “torn shoulder,” these injuries are commonly associated with deep shoulder pain, arm weakness, difficulty reaching behind your back, and trouble sleeping due to discomfort. Surgery may be needed depending on severity, but appropriate physical therapy can often reduce pain and restore rotator cuff strength without surgery. If surgery is required, physical therapy remains an important part of the recovery process.
